Causa and the affiliate Associations that it is aligned with, offer Membership packages (Programs) with the following terms and conditions of Membership:

CAUSA MEMBERSHIP AGREEMENT & QUALIFIED USERS:

Except as provided for in these Terms and Conditions, You, the Member, must be actively enrolled for at least fifteen (15) days and current on your membership fees for the Program to receive the privileges of your membership. You agree that you will use your membership only for your personal benefit or the benefit of your Household Members. "Household Members" are immediate family members living with you or family members living with you that are financially dependent upon you as per the IRS definition of dependent.

LIMITED MONEY-BACK GUARANTEE:

Your Membership is monthly renewing. You will be notified by direct mail or email of your activation as a Member. If you cancel in the first 30 days of your Membership, you will receive a full refund of your monthly Membership amount. Some Programs require written cancellation; contact the Member Services toll-free number listed in your Program materials for specific instructions for cancellation. Notwithstanding the above, any portion of the membership fees that represent earned premiums, are not refundable.

MEMBER COMPLAINTS:

Members may file a grievance regarding any aspect of their Program by contacting the Program Member Services toll-free number listed in their membership materials, or by sending an email to QUALITY@TEAMCAUSA.ORG. The CAUSA QUALITY ASSURANCE team will respond with an answer within ten business days of the initiation of the complaint.

CHANGE IN MEMBERSHIP TERMS:

Subject to applicable law, the Program Manager reserves the right to change or terminate any term, condition or benefit of the membership without limitation. Prior notice will be provided to you when required by applicable law.

LIABILITY:

The Companies, including the Program Manager, the Employer, the Networks, the Association, and the Program Marketer, assume no liability with regard to any services provided, or to be provided, by any participating Providers. All Medical Providers are independent contractors of the Program, and are solely responsible for the professional advice and treatment rendered to Members. You agree that any claim with regard to services shall be made against the Providers of such services and not the Companies. In the event that any product or service purchased or received by You is cancelled, modified, defective, or otherwise unsatisfactory to You, You will look solely to the Providers, Seller, Merchant, or Manufacturer of the product or service for any repair, exchange, refund, or satisfaction for the claim. It is further understood that all services requested are subject to the availability of such services and any information or benefits provided to you are subject to change without prior notice.

ASSOCIATION MEMBERSHIP BENEFITS:

One of the privileges of your Membership is enrollment in the Program's affiliate Association(s), with access to certain guaranteed issue insurance coverages for you and your household Members. Information on these Association policies is available through your Program Manager by calling Member Services at the toll-free number listed in your membership. Your affiliate Membership in the Association is on a month to month basis. Upon termination of your Program Membership for whatever reason, your affiliate membership in the Association(s), will be terminated, and any claims or services subsequent to this date will be declined.

RESPONSIBILITY FOR PAYMENT FOR MEDICAL SERVICES:

You the Member agree that you are solely responsible for payment of any net amounts due for medical services rendered, whether upon receipt of medical service or promptly (within 10 days) after presentation of a net re-priced bill for medical services. Failure to make prompt payment of the net amounts due to the Medical Provider will result in the loss of the Membership medical discount, and may result in loss of Membership privileges and submission to collections for the full billed amounts. Neither the Program Manager, the Employer, the Networks, nor the Program Marketer shall be considered either a guarantor or payor for any medical treatment rendered to any Member. Discounts on professional services are not available where prohibited by law. Services and health care Providers may change or be discontinued at any time without notice. DISCLAIMER OF WARRANTIES:

THE COMPANIES ARE NOT MERCHANTS, MANUFACTURERS OR DIRECT PROVIDERS OF THE SERVICES AVAILABLE TO YOU. ACCORDINGLY, WE DO NOT GIVE ANY WARRANTIES, EXPRESSED OR IMPLIED, AS TO DESCRIPTION, QUALITY, MERCHANTABILITY, FITNESS FOR ANY PARTICULAR PURPOSES, PRODUCTIVENESS, OR ANY OTHER MATTER, FOR ANY SERVICES OR MERCHANDISE PURCHASE OR RECEIVED BY YOU OR PROVIDED THROUGH MEMBERSHIP IN THE PROGRAM. YOU ACKNOWLEDGE THAT YOU ARE NOT RELYING ON THE COMPANIES SKILL OR JUDGEMENT IN SELECTING A PROVIDER FOR THE SERVICES AVAILABLE TO YOU.

GENERAL RELEASE:

You and Your Household Members hereby forever release, acquit and discharge each of the Companies and their respective officers, directors, employees and agents from any and all liabilities, claims, demands, actions and causes of action that You or Your Household Members may have by reason of any monetary damage or personal injury sustained as a result of, or during the cause of any and all services provided under the Program. The sole recourse available to you or your Household Member from the Program shall be cancellation of the Association Program membership, and refund available as provided in the LIMITED MONEY-BACK GUARANTEE section of these terms.
